Obtaining a duplicate of the driving license

• Copy of the complaint lodged at the police within 6 months of the loss of driving license.
• National Identity Card or the valid passport with the national identity card number
• The distorted / defaced driving license, if available.

Change of Particulars

• Current driving license.
• National Identity Card or the valid passport with the national identity card number
• Birth certificate / marriage certificate / Divorce  certificate or any other valid document for changing the name.
• Grama Niladhari Certificate for altering the address
